Any insurance card(s) currently in effect
Your driver’s license or another form of valid photo identification
A record of your current prescription medications and vitamin supplements
Your medical records from your former doctor or primary care physician; if not available, you can request your previous doctor to fax them to us at 502-947-2274
Your health history, encompassing chronic conditions, illnesses, and prior surgeries
A list of the specialists you visit (names and contact information)
A set of questions you wish to inquire about during your visit.
