  • Annual Physical (Include Labs & Preventive Health Screening Referrals)
  • Office Visits (30 minutes & Diagnostic lab testing with some exceptions)
  • Specialist Referrals
  • DOT (Department of Transportation)
  • Sports Physicals
  • Access to provider using portal
FLAT RATE FEE SERVICE (Non-Members)
  • Annual Visit (Include Labs & Preventive Health Screening Referrals): $270.00
  • Office Visits (30 minutes): $75.00
  • Well Woman Exam (Include Labs, Pap Smear, & Preventive Health Screening Referrals): $280.00
  • Sports Physicals: $50.00
